Useful Forms

We have one main form that we use for everyone at our initial consultation. We use it to obtain information about you, our potential client, your home and your pet(s). It also includes a veterinarian information and release form that should be filled out in case of an emergency. To save time at the initial consultation, please fill out the forms prior to our meeting, Please fill out a separate pet profile for each pet (yes, even if we are walking your dog and not caring for the cat- we need to know about all animals in the home we may interact with).

Pet Sitting Form

There are two other forms that you may find useful to print and fill out before our initial consultation. The first is the Credit Card Release Form, which may be used to authorize payment for emergency medical treatment, or to set up automatic payment of invoices  The second is the Vacation Itinerary and Contact Information Form.  This form tells us where you will be while out of town, when you are leaving and returning, and whether you would like to receive email updates about your pet(s) while away.
